Terms of Boarding:
All animals must be current on all required vaccinations including Rabies, DA2PP, and BB for canines and Rabies, FVRCCP for felines. All vaccinations must be given by a licensed veterinarian. If I do not provide proof that the required vaccinations are current at boarding check-in, I authorize North Florida Animal Hospital to complete an exam and administer the necessary vaccines at my expense.
If any internal or external parasites (fleas, ticks, tapeworms, roundworms, etc) are found on my pet at any point during their boarding stay, I authorize North Florida Animal Hospital to treat my pet at my expense.
If North Florida Animal Hospital is unable to contact me at the given phone numbers I authorize any treatment for my pet that the doctor considers necessary for his/her health and safety. I understand that I will be responsible at discharge for any charges incurred for treatments performed.
I will not hold North Florida Animal Hospital responsible for inadvertent loss or damage of personal items left with my pet.
Full payment is required at the time of check out. Prepayment is acceptable for any boarding and required for Sunday Boarding Pick Up.
